《最终用户和最终用途证明》的内容和格式

企业在申请时必须提交《最终用户和最终用途证明》的正本1份。证明必须由真正的最终用户出具，由其主要负责人（董事长、副董事长、总裁、副总裁、总经理、副总经理等）亲笔签字（圆珠笔、铅笔签字无效，印鉴、手签章无效），并打印注明签字人全名和职务，加盖最终用户企业印章。签名和企业印章必须完整清晰。注意填写签字日期。出口商负责提供中文译本，并加盖出口商企业印章。《最终用户和最终用途证明》的英文和中文译本格式和香港、澳门、台湾地区专用的《最终用户和最终用途证明》的格式如下：
